Upload a batch of changes

At most 500 changes and 5242880 bytes. Per record, the latest changed_at wins; a time ahead of the server's is capped at it, and a tie keeps the change that arrived first. A change dated by the device rather than by the record (a deletion, say) must keep its changed_at when it is sent again.
